Abstract

Objective. To develop the method of treatment of degloving soft tissue injuries complicated by crushing of fascia and muscles.

Material and methods. There were 40 patients with extensive detachments of integumentary tissues who underwent original surgical treatment. This technique was based on excision of non-vascularized exfoliated skin flaps after revision of wounds with exfoliated integumentary tissues, the underlying muscles and fascia. Next, their special processing is performed. Replantation of full-layer skin flaps is carried out if the wounds are ready for reconstructive closure. A new aspect is consisted in wound closure using a full-layer skin graft during the second surgery. This graft was preoperatively stored in refrigerator at 3-4°C immersed in saline solution.

Results. We observed primary wound healing within a short period. There were no soft tissue inflammation and necrosis in early postoperative period.

Conclusion. Two-stage treatment of degloving soft tissue injuries consisting in preparing the recipient area and preserving the full-layer skin grafts ensures primary wound healing in most cases.
