Abstract

Objective. To ensure synergy of high PMAS mobilization and facial tissue mobilization in deep plane by preserving the main lymphatic collectors of lateral surface of the face in combination with effective fixation of SMAS flap to deep temporal fascia.

Material and methods. A feature of original method is zigzag cutting of SMAS with further mobilization towards modiolus, lifting and fixation to deep temporal fascia. Ther were 35 patients after this surgery. In this case, the main lymphatic drainage pathways of lateral surface of the face remain intact. This is a predictor of fast rehabilitation.

Results. Original method has all advantages of high SMAS and mobilization in deep plane. Among 35 patients, difficulties in mobilizing the SMAS were observed in 2 patients due to multiple thermal effects (Altera, Thermage). Postoperative period was uneventful, and wounds healed via primary intention.

Conclusion. Original method solves two important problems - preservation of the main lymphatic collectors of lateral surface of the face and simultaneous fixation of SMAS to deep temporal fascia. This leads to stable results and rapid rehabilitation.
